Molecular profiling reports from most labs are static, reflecting clinical knowledge only at the time of testing. As new biomarker–drug associations and therapeutic indications emerge, patients with previously identified biomarkers may miss newly approved treatment opportunities, while clinicians face increasing challenges staying current with evolving evidence.Since 2014, Caris patients have benefited from the dynamic Caris Lookback Program, which addresses limitations of static molecular profiling reports. PR Newswire (US) Targeted gene panels miscalculate tumor mutational burden in 10–15% of patients, directly resulting in incorrect pembrolizumab eligibility determinationIRVING, Texas, May 11, 2026 /PRNewswire/ -- Caris Life Sciences® (NASDAQ: CAI), a leading, patient-centric, next-generation AI TechBio company and precision medicine pioneer, has published a study in Cancer Immunology, Immunotherapy demonstrating that measuring tumor mutational burden (TMB) using ultra-deep Whole Exome Sequencing (WES) provides superior prediction of pembrolizumab immunotherapy benefit compared to estimates of TMB from targeted gene panels. TMB is a pan-tumor biomarker used to determine patients' eligibility for pembrolizumab. These findings highlight the importance of testing all cancer patients with ultra-deep WES, the only truly comprehensive genomic profile for therapy selection. The study used Caris' large-scale, real-world clinico-genomic database, containing 26,756 patients treated with pembrolizumab who were evaluable for this study. WES provides a true measurement of TMB by interrogating every protein-coding gene mutation that may create a neoantigen, in comparison to targeted panels that only estimate TMB with incomplete gene coverage.Key findings include:The analysis compared WES-measured TMB with commercially available targeted panel estimates of TMB and found discordance in 10-15% of cases, with error rates correlating to panel size.In discordant cases, WES TMB more accurately predicted overall survival in pembrolizumab-treated patients than panel-based estimates.In a subset of 'TMB reliant' patients (n = 3,981), for example, patients with tumor types that lack disease-specific immune checkpoint inhibitor indications, the median overall survival in discordant cases was about five months longer for WES TMB-High and panel TMB-Low compared to WES TMB-Low and panel TMB-High cases treated with pembrolizumab."These findings underscore the critical importance of using Whole Exome Sequencing to guide immunotherapy decisions," said Milan Radovich, PhD, Senior Vice President, Chief Scientific Officer at Caris. Caris Assure is a blood-based molecular profiling test designed to support comprehensive biomarker analysis using a minimally invasive blood sample. Caris Assure uses circulating nucleic acids sequencing (cNAS) to analyze the whole exome (DNA) and whole transcriptome (RNA) of 22,000 genes. Research published in JCO Oncology Practice found that 70% of U.S. counties had no active cancer treatment trials, leaving nearly one in five Americans ages 55 and older without a local pathway to investigational therapies. Nearly 85% of U.S. cancer patients receive care at community-based practices, yet most clinical trials remain concentrated at large academic medical centers. This study builds on the body of evidence showing the superiority of Caris' proprietary and world-leading GPSai® algorithm over traditional diagnostic procedures in correctly diagnosing cancer types. 







This paper demonstrates that by including AI algorithms as part of routine comprehensive molecular profiling, clinicians can uncover clinically significant misdiagnoses in cases labeled as lung squamous cell carcinoma (SCC), influencing treatment decisions and patient outcomes. The study comprised 3,958 lung cancer cases submitted to Caris with a diagnosis of SCC. Caris GPSai® identified 123 of the 3,958 cases were metastases from other primary sites, including cutaneous, urothelial, head and neck and thymic cancers.The clinical impact of reclassification underscores the power of precision medicine. The study identified 88 patients (71.5%) who had guideline-preferred first-line systemic therapy change recommendations, alerting clinicians that a change in their patients' treatments would achieve better outcomes. According to the CDC website, lung SCC accounts for roughly 21% of all lung cancer cases in the United States. This interim readout of Achieve 1 represents a major milestone in Caris' goal to detect cancer earlier, when it is most treatable, through Whole Genome Sequencing, advanced AI and molecular insights.







Caris Detect leverages Caris' industry-leading molecular profiling data, which has surpassed one million cases processed and produced more than 50 billion molecular markers. This deep molecular foundation enables Caris' AI models to identify subtle biological signals associated with early-stage cancers with unprecedented resolution. Achieve 1 is evaluating Caris Detect across a broad population of patients, assessing its ability to detect multiple cancer types at early stages using a blood draw. For the undiagnosed cohort, which consisted of 1,505 samples, we utilized blood draws from subjects who had screening or symptomatic screening, a population reflecting a higher likelihood of cancer than the general population. We followed 22.5% of the subjects for approximately one year following their blood draw. Of these, 121 subjects had no symptoms of cancer, no significant risk factors of cancer and were not subsequently diagnosed with cancer, representing a healthy population.The first phase of Achieve 1 utilized a cross-validation approach, with approximately 865 samples held out for a blinded validation that is in process. This interim readout of Achieve 1 represents a major milestone in Caris' goal to detect cancer earlier, when it is most treatable, through Whole Genome Sequencing, advanced AI and molecular insights.







Caris Detect leverages Caris' industry-leading molecular profiling data, which has surpassed one million cases processed and produced more than 50 billion molecular markers. This deep molecular foundation enables Caris' AI models to identify subtle biological signals associated with early-stage cancers with unprecedented resolution. Achieve 1 is evaluating Caris Detect across a broad population of patients, assessing its ability to detect multiple cancer types at early stages using a blood draw. For the undiagnosed cohort, which consisted of 1,505 samples, we utilized blood draws from subjects who had screening or symptomatic screening, a population reflecting a higher likelihood of cancer than the general population. We followed 22.5% of the subjects for approximately one year following their blood draw. Of these, 121 subjects had no symptoms of cancer, no significant risk factors of cancer and were not subsequently diagnosed with cancer, representing a healthy population.The first phase of Achieve 1 utilized a cross-validation approach, with approximately 865 samples held out for a blinded validation that is in process. IRVING, Texas, Jan. 28, 2026 /PRNewswire/ -- Caris Life Sciences® (NASDAQ: CAI), a leading, patient-centric, next-generation AI TechBio company and precision medicine pioneer, has published a study in Nature's npj Breast Cancer journal titled, "Mechanisms of Resistance to Trastuzumab Deruxtecan in Breast Cancer Elucidated by Multiomic Molecular Profiling." Utilizing large-scale real-world clinico-genomic data and Caris's comprehensive multiomic profiling, Caris scientists uncovered clinically relevant mechanisms of resistance to trastuzumab deruxtecan (T-DXd) in metastatic breast cancer. This study explains why patient responses vary and reveals how resistance can evolve over time.







This study addresses a critical gap in understanding why responses to T–DXd, an antibody–drug conjugate approved by the FDA for HER2-positive and HER2-low metastatic breast cancer, vary widely since resistance to T–DXd is common. By combining large-scale, real-world clinical outcomes with Caris's comprehensive molecular profiling, including whole exome sequencing (WES) and whole transcriptome sequencing (WTS), the study enables a population-level analysis of resistance pathways across DNA, RNA and protein expression."In this large real-world analysis, clinical outcomes allowed Caris to pinpoint molecular features linked to treatment-specific survival,"?said George W. Sledge, Jr., MD, Caris EVP and Chief Medical Officer.?"Post–treatment molecular shifts further reveal biologically plausible routes to acquired resistance, underscoring that RNA–level and multiomic profiling can provide actionable insights beyond standard HER2 classification to better stratify patients and inform therapies." The study analyzed 2,799 T–DXd–treated breast cancer patients and found that WTS identified ERBB2 (HER2) and ABCC1 as the strongest transcriptomic predictors of T–DXd–specific overall survival. Higher ERBB2 expression correlated with improved outcomes, while higher ABCC1 correlated with poorer outcomes, independent of HER2 category.ABCC1 further stratified outcomes within HER2–defined subgroups, indicating predictive value beyond standard HER2 testing and post–treatment samples showed increased ABCC1 expression alongside enrichment of mutations in ERBB2, NFE2L2, KEAP1 and TOP1. Consistent with acquired resistance mechanisms.
