The Ensign Group, Inc. is a holding company. As of December 31, 2010, the Company, through its subsidiaries (collectively, Ensign), provided skilled nursing and rehabilitative care services through the operation of 82 facilities and one home health and hospice operation, located in California, Arizona, Texas, Washington, Utah, Colorado and Idaho. All of these facilities are skilled nursing facilities, other than three stand-alone assisted living facilities in Arizona, Texas and Colorado and five campuses that offer both skilled nursing and assisted living, independent living or hospice care services located in California, Arizona and Texas. Effective December 1, 2011, the Company acquired Rosewood Rehabilitation Center. In January 2012, the Company acquired Orem Rehabilitation & Nursing Center. On January 1, 2012, the Company acquired Rose Court Senior Living. On February 1, 2012, the Company acquired Park Place Assisted Living, a 60-unit assisted living facility in Reno, Nevada.
