A critical AI success gap is emerging for organizations, with 30% surveyed seeing an impact on new revenue streams.ORLANDO, Fla., April 30, 2026 /PRNewswire/ -- Most organizations have moved beyond experimenting with artificial intelligence, but few are realizing its full value. New research from Harvard Business Review Analytic Services, sponsored by Appian, finds that while 59% of organizations (who are moving forward with AI to some extent) have AI in production, the majority are currently focused on incremental gains that prioritize efficiency and productivity over top-line growth.
Notably, AI has the strongest impact in bolstering productivity, not enabling growth. Respondents indicated that of the AI performance measures their organization tracks, most see impact in productivity improvements (64%) and operational efficiency (58%), while metrics like new revenue streams (30%) and ROI (35%) are among the least likely to have improved. This points to a significant opportunity for organizations to use AI to deliver broader business outcomes and growth."Enterprises are at an inflection point. Instead of using AI to drive productivity, organizations must evolve to focus on business growth. That's where Appian comes in," said Matt Calkins, CEO of Appian. "The true potential of AI can only be realized when it moves from a standalone tool to an embedded worker that drives revenue. Only 18% of respondents report that AI is primarily integrated within workflows, while a larger share (34%) continue to use AI as standalone tools alongside processes/workflows, with another 34% reporting a mix of both approaches and 12% not yet using AI in processes/workflows at all.Most see some returns on AI, but not yet at scale Most respondents are seeing some returns from AI investments, but only 16% report realizing a high degree of measurable value. The majority describe the impact as moderate (33%), slight (36%), or have no measurable value (8%). Still, expectations remain high, as 86% agree that their organization is looking to realize more business value from its use of AI. It's clear that AI is delivering some results, but translating those results into meaningful, scalable business impact is proving difficult.AI delivers value when embedded in workflowsAs organizations advance their AI strategies, value is closely tied to how effectively AI is integrated into workflows and applied to operational work. Seventy-one percent of organizations embedding AI into processes realized substantial or moderate value from those efforts, according to respondents. In parallel, approximately three-quarters report strong returns from modernizing legacy infrastructure/systems (76%), integrating data sources (75%), and orchestrating processes/workflows across systems/applications (73%).Legacy systems continue to limit AI's impactNearly seven in ten respondents, 69%, agree that legacy systems are limiting their ability to scale AI across the enterprise. This reinforces the need for modernization and better integration across systems and data. Siloed or low-quality data (34%), a lack of integration across systems (31%), and a lack of AI talent/skills (30%) are also among the most commonly cited barriers to embedding AI into workflows.AI agent adoption lags in core operationsThe research also highlights differences in how AI agents are being applied across the enterprise. Organizations are more actively deploying AI agents in areas such as software development (35%), IT operations (31%), marketing and sales (26%), and customer service (25%). In contrast, agent adoption is more limited in core operational areas such as procurement (9%), manufacturing (10%), and supply chain (11%), where processes tend to be more complex and require greater control and consistency. As organizations look to expand AI into these environments, governance becomes critical.Most organizations lack the guardrails needed to scale AI agents safelyNinety-two percent of respondents agree that AI agents need rules-based guardrails to operate safely and effectively, yet fewer than half (48%) agree that their organization has defined such rules (among those at organizations using, considering or exploring agentic AI). As organizations explore agentic AI systems (currently used by 25% of organizations and under consideration by 62%), the need for clearly defined processes and guardrails will become even more critical. Without clear guardrails, agents can act unpredictably across systems, increasing the risk of unintended outcomes.Process design is emerging as the key to unlocking AI valueRealizing the full value of AI and achieving sustainable ROI requires rethinking how work is structured and governed. 63% trust NHS in the use of AI in public services, the only public sector body with a net trust over 50%LONDON, Feb. 26, 2026 /PRNewswire/ -- New research from Appian (Nasdaq: APPN) shows the NHS is the most trusted organisation for responsible AI use, ranking higher than any other public or private sector organisation, including banks, retailers, and consumer tech firms.
The 2026 UK Public Sector AI Adoption Outlook, conducted by Censuswide on behalf of Appian, polled 1,000 UK public sector workers and 1,000 UK citizens. It reveals that the NHS stands apart from the rest of the public sector when it comes to public trust in the use of AI. The NHS is trusted by 63% of citizens to use AI responsibly—more than any other public sector organisation and the only one with a net trust over 50%.By contrast, fewer than half of UK citizens trust central government (39%) or local councils (44%) to use AI responsibly. The NHS also ranks higher than consumer-facing organisations, including banks (55%), retailers (60%), and technology companies (54%).High trust overall but uneven comfort with clinical AIWhile trust in the NHS to use AI responsibility is high, comfort with specific AI use cases is more mixed. Over half of public sector workers (56%) say they are comfortable with AI analysing NHS scans and diagnostics, compared with just 40% of citizens sharing that sentiment. The findings highlight a gap between institutional trust in the NHS and public confidence in the use of AI for clinical decision-making.NHS seen as best place to benefit from AI useOne year on from the AI Opportunities Action Plan, where the Government allocated £2bn to implement AI research and resources, the new Appian research found that the NHS is the part of the public sector most likely to benefit from AI investment, with 30% of public sector workers and 29% of citizens agreeing.When asked how they hope to benefit from integrating AI into public services, citizens placed faster services and shorter wait times (35%), improved safety (26%), and easier-to-use digital services (26%) at the top of the list.Patient communication should be a priorityThe findings in the report highlight that NHS organisations should pair any clinical and non-clinical AI rollout with clear explanations, human oversight and demonstrable safety measures—especially at a time when public understanding of how AI is used in the public sector remains low.In fact, 75% of citizens cannot name a single way the government currently uses AI, suggesting a major communications and transparency opportunity for government to publish accessible explanations of where and how AI is used.Is the NHS ready?Despite strong public trust, there are relatively low levels of organisational readiness within the NHS, with just 6% of respondents working in healthcare describing their organisation as "fully able" to leverage AI, signifying a substantial delivery gap.A majority of respondents say process readiness and "fixing basic processes" should be a priority over AI implementation, with both public sector workers and citizens agreeing (55% and 56%, respectively) that this should come before introducing new AI technologies."The NHS is widely seen by both public sector workers and citizens as the area of public services most trusted and likely to benefit from AI," said Peter Corpe, Industry Leader, UK Public Sector at Appian. Some adopt it as a bolt-on experiment (23%) or as a standalone work tool (22%), with little or no integration with existing core processes. This suggests that 45% of government AI applications are not implemented as part of an integrated process improvement, creating potential challenges for delivery and impact. This lack of integration into existing processes was one of the reasons MIT found that 95% of AI investments have produced no return in a 2025 report. Corpe concludes: "AI delivers when it's given a job inside a governed process which moves it beyond experimentation into integrated, measurable programmes. The advice is clear to healthcare leaders: Give AI the right data, guardrails and purpose, and organisations will deliver outcomes that are safe, auditable and easier for the public to get behind. The 2026 UK Public Sector AI Adoption Outlook report, which surveyed 1,000 public sector workers and 1,000 UK citizens, reveals a clear divide between those tasked with delivering AI-enabled services and those who use them. While two thirds (67%) of public servants believe AI will improve public services over the next five years—rising to 87% among director-level leaders—only 44% of citizens share this optimism, a figure closely mirrored by workers in administrative roles (40%).This disconnect could be explained by the way AI is currently being deployed inside government. With nearly half (45%) of AI initiatives operating as bolt-on experiments or standalone tools rather than being embedded into core service workflows, many applications remain invisible to citizens—limiting public awareness of where and how AI is already in use."Too much AI in the public sector is still being used as a personal productivity tool rather than embedded into the processes that actually run services," said Peter Corpe, Industry Lead UK Public Sector at Appian. If organisations want AI to move beyond pilots and produce real value, it has to be integrated into core processes from the start."Public trust in AI remains limited Public trust in responsible AI use remains low across much of government, with fewer than half of UK citizens trusting central government (39%) or local government (44%) to use AI responsibly—placing government behind retailers (60%), banks (55%) and consumer technology companies (54%). The clear exception is the NHS, which commands a 63% net trust rating, making it the most trusted organisation for AI use across both public and private sectors.Regarding AI making decisions without human oversight, 67% of public sector workers are comfortable with AI selecting cases for tax or benefits compliance checks compared with 40% of citizens, while 56% of public sector workers support its use in analysing NHS scans versus 40% of citizens. Concerns about AI also extend beyond individual decisions, with the majority of the public worried about implications around data security and privacy (67%), job losses (63%), auditability of decisions (61%) and ethical oversight and bias (59%).Fixing processes should come before delivering AI at scale Inside government, enthusiasm for AI is tempered by concerns about execution. Less than a third (29%) of public sector workers say their organisation or department is delivering on most of its AI commitments. A similar proportion say they are moving slower than planned (27%), while a quarter (25%) identify a significant gap between AI strategy and delivery.One year on from the AI Opportunities Action Plan, where the Government allocated £2bn to implement AI research and resources, the new research findings point to a growing disconnect between strategic ambition and service delivery reality. Nearly 9 in 10 public sector workers (89%) say their organisation is not fully able to leverage AI.This delivery challenge is widely recognised by both public sector workers and citizens. A majority of public sector workers (55%) and citizens (56%) agree that existing processes must be fixed before new AI technologies are introduced, prioritising process improvement over deploying new AI tools."AI is only as good as the work you give it," said Corpe. Citizens want AI investment to deliver faster services (35%), improved public safety and fraud prevention (27%) and easier-to-use digital services (26%). By contrast, public sector workers are more focused on efficiency gains (47%) and cost savings (41%), highlighting that citizens focus on outcomes they directly experience and public sector workers focus on how those outcomes are delivered.
